We are very excited to be a part of the effort to vaccinate our community against COVID-19. Initially, our actual patients are not included in the priority groups to be vaccinated first, but using our resources to help get vaccine distributed to others in those priority groups is important to us.

As we receive vaccine allocations from the state of Tennessee, we will open spcial vaccine appointment times and reach out to individuals who are on our waiting list to schedule appointments.

You may use the form below to be added to the list. We will be prioritizing our utilization of our allocated vaccines to higher risk groups first and then by order that the requests were received.
