If you’re new to our area or switching from another practice, it may be time for your child to have a regular wellness visit.  If so, that’s a great way to meet your new pediatrician and our staff.

If it’s not time for a regular checkup visit but your child needs to come in because of an illness or injury, go ahead and make an appointment for that visit.  We don’t insist on your first visit being a wellness visit.

When you’re ready to schedule your first appointment, please go to our patient portal and click on “Preregister” to enter your child’s basic information.  Then call our office to schedule the appointment.  You should have your insurance and preferred pharmacy information available if possible.

We want to make sure that we are providing consistent care for your child and that involves knowing the medical history and having the vaccine record.  You can use our medical records release form to request records from your former pediatrician. If you need help with this process, please call our medical records department.

Before your child’s first visit, it is very important the we have a copy of the vaccine record.

When you arrive for your visit, there will be some paperwork to do.  If you would like to do that ahead of time, you may download those forms and bring them with you to the office.
