Medical Record Release Authorization
Use this form when requesting or sending copies of medical records. It allows documents to be transferred from another physician’s office to Laurelwood Pediatrics or to obtain a copy of your child’s records from our office.
The Vanderbilt Parent Assessment helps evaluate learning, attention, or ADHD concerns. Parents should complete this form prior to the appointment.
ADHD Evaluation Form – Teacher
The Vanderbilt Teacher Assessment provides additional insight for ADHD evaluations. Please have your child’s teacher complete this form before the visit.
Adult Self-Report Scale (ASRS) for ADHD
This form is used to assess ADHD symptoms in older teens and college-aged patients and may be requested before consultation appointments.
